UnitedHealthcare offers special needs Medicare Advantage plans that provide full Medicare health care coverage and prescription drugs, as well as additional benefits and services for people who have long-term or advanced illness, are older, or have disabilities
UnitedHealthcare Special Needs Plans (SNP) include:
- UnitedHealthcare Dual Complete - Offers benefits geared toward people with both Medicare and Medicaid living in community settings. Plans provide more benefits than Original Medicare, such as transportation to medical appointments and vision exams. UnitedHealthcare also helps coordinate Medicare and Medicaid benefits for members.
- UnitedHealthcare Nursing Home Plan - Provides individualized, closely monitored care to nursing home residents in UnitedHealthcare contracted facilities. UnitedHealthcare Nurse Practitioners work with facilities, members, and the member's family to coordinate care across settings.
- UnitedHealthcare Chronic Complete - Benefits and services tailored to meet the needs of people living with specific chronic conditions including diabetes, cardiovascular disease and congestive heart failure. In addition to preventive care and other benefits not covered by Original Medicare, members are aligned with programs to help coordinate care and keep them healthy.

If you are a participating provider for Medicare plans in a contract with UnitedHealthcare and/or its affiliates there is nothing additional that you need to do to participate in these plans.
Although the process of integrating some of the key administrative systems continues, member data is currently housed in multiple systems. Please continue to check benefit plan information, eligibility, and claims status through the same website(s) you are accustomed to using based upon your contractual relationship with UnitedHealthcare and/or its affiliates or call the phone number listed on the back of the member Identification Card.
